Substance Use Evaluations are $300 out of pocket. We require a $200 deposit due at the time of scheduling and the remaining $100 balance is due at the time of service. This deposit covers the late cancel/no-show fee, should this occur.
This deposit can be refunded ONLY if cancelations/reschedules are initiated prior to 8am on the day of service (per our Late Cancel / No-Show Policy)
We do not bill insurance for this service, and can provide a super bill that can be submitted to insurance for out of network coverage.
We may be able to offer flexible pay plan options upon request.
The majority of the time, a SUE is required by the courts as a part of probation, pre-trial, or ongoing legal matters. These appointments may be sought voluntarily by anyone wanting to know more about the impact of substances in their life.
The SUE appointments follow the structure outline below:
Initial paperwork - completed prior to appointment
Clinical interview - 1.5 to 2 hours
Self-report Assessments - 20-30 min
A written report with assessment results and clinical recommendations will be provided within 2-3 weeks after the SUE appointment is completed.
